Use the records request form to have the following information sent directly to Open Window School by January 19, 2012.
- Grades/transcripts from the current year and the previous 2 years
- Standardized test results
- Discipline Records
Open Window School requires two (2) teacher evaluations for Middle School applicants. One from the applicant’s current teacher and one from the previous year’s teacher. Copies of the Teacher Evaluation form may be downloaded here.
