Use the records request form to have the following information sent directly to Vista Academy by January 31, 2008
- Grades/transcripts from the current year and the previous 2 years
- Standardized test results
- Discipline Records
Vista Academy requires two (2) teacher evaluations. One from the applicant’s current teacher and one from the previous year’s teacher. Copies of the Teacher Evaluation form may be downloaded here.
